Community Health Plan of Imperial Valley (CHPIV) is Imperial County's Medi-Cal managed care plan. We are a locally managed public health care plan committed to working with members, providers, and the residents of Imperial County.


Position Overview
Our team is growing, and we’re looking for a part-time clinician to provide home-based health assessment services for our Medicare Advantage Prescription Drug (MAPD) members to support risk adjustment and quality of care. 

This is a flexible, part-time contract position. Contractor retains full discretion over the method, schedule, and manner in which the Services are performed, consistent with professional standards and regulatory obligations. Contractor is not eligible for employee benefits, unemployment insurance, or workers’ compensation.

Key Responsibilities
 
  • Conduct yearly in-home health visits for CHPIV Medicare members who have not had an annual wellness exam with their primary doctor.  These visits last about 45-60 minutes and include various evaluations and tests:
    • Initial Assessment: Review of current symptoms, medical history, and medications to understand overall health status.
    • Physical Examination: Measure vital signs such as blood pressure, heart rate, temperature, and respiratory rate. May also include a check of the patient’s lungs and heart, examination of specific complaints, and assessment of reflexes.
    • Preventive Screenings: Depending on age and risk factors, screen for conditions like cardiovascular risk, diabetes, cognitive impairment, depression, or fall risk. Eye and foot examinations may also be included.
    • Medication Review: check prescriptions to identify potential interactions, confirm dosages, and help with refills if needed.
    • On-Site Diagnostics: Point-of-care tests such as blood draws, ECGs (heart monitoring), blood sugar testing, or other laboratory tests, when available and appropriate.
    • Assessment of Living Environment: Evaluate home for safety hazards that could affect health, such as fall risks or mobility challenges.
    • Health Counseling: Discuss diet, nutrition, exercise, and chronic disease management, as needed.
  • Care Coordination: Communicate with the patient’s primary care physician (PCP) and CHPIV care manager, as needed to support follow-up of recommended care plan or clarification of patient’s health data.
    • Documentation - Documenting assessments and diagnoses associated with accurately in CHPIV’s electronic health record (EHR) system.
    • Coding diagnoses consistent with CMS guidelines and risk adjustment protocols based on Hierarchical Condition Categories (HCCs).
  • Compliance:  Maintain compliance with all federal, state, and local laws, including HIPAA, and applicable regulatory requirements.

Qualifications
  • Licensed in California and in good standing as a Nurse Practitioner, Physician Assistant or Doctor of Medicine.
  • Contractor shall maintain professional liability insurance (malpractice) and other insurance as required under the applicable state law and professional regulatory standards. Contractor shall provide certificates of coverage to the Client upon request.
